Leslie Taylor is a board-certified psychiatric mental health nurse practitioner (PMHNP-BC), trained to provide care across the lifespan. She utilizes holistic and evidence-based practices to help guide individuals through difficult feelings and situations related to anxiety, depression, PTSD, personality disorders and neurocognitive disorders, such as dementia and Parkinson’s disease. She is passionate about providing high-quality mental health care to those who are suffering. She provides medication management, psychotherapy services, screening and lab testing, neurocognitive screening and management, hydration and nutraceutical education and support, as well as consultation/guest speaker services as a mental health advocate in Southwest Florida and East Tennessee communities. SWAG Mental Health Services was founded by Leslie and her business partner and father, Richard Taylor, a retired minister and a successful business owner for over three decades. Our practice was established on the SWAG Model of Care. This unique framework integrates education, holistic interventions, and faith-based healing to provide comprehensive mental health support.

Those who struggle with mental health concerns often withdraw from their community. Our team's holistic approach treats the whole person by healing the mind, body, and spirit, working with adults to examine all aspects of the individual: the psychological, spiritual, social, and biological. We strive to fully understand each person's unique challenges and facilitate healing through a multi-disciplinarian approach.
